The GHCHA directory, accessible on this website, lists facilities and providers selected to participate in the GHCHA network. Although the directory is regularly updated, changes which affect the directory listings may occur at any time, and GHCHA is not responsible for any resulting and/or inadvertent errors or omissions in the directory listings. The directory is provided to allow GHCHA clients to better determine which health care facilities and providers are participating in the GHCHA network, but is not intended or offered as a final confirmation of the current participating status of any facility or provider.

Not all GHCHA network facilities and providers participate with each health plan, PPO program and client of GHCHA. Therefore, we strongly recommend that each insured/patient confirm that a facility or provider is currently participating in the insured's/patient's health plan/PPO program prior to receiving services from that health care facility or provider. Participation information may be obtained by calling the particular facility or provider directly, or by calling one of the GHCHA offices listed in this website.

The decision as to which health care facility or provider to use and which procedures/treatments are performed is the sole responsibility of the insured/patient. In making this decision, the insured/patient must understand that: (1) selection of an out-of-network facility or provider (whether intentionally or unintentionally) could waive entitlement to benefits listed in his/her health plan; while (2) selection of an in-network facility or provider does not guarantee that all services provided by an in-network provider will be covered under the insured's/patient's health plan. Again, GHCHA strongly recommends that insured's/patient's verify covered benefits from their health plan provider prior to receiving health care services from ANY health care facility or provider.
